Dear Claremont Bank Surgery Patients,
New Long term conditions recall system
We are introducing a streamlined system for annual reviews of long-term conditions (e.g., diabetes, asthma), to align recall invitations to your birth month.
How it will work:
- You’ll receive an invitation via text or letter just before your birthday month.
- The invitation will include details on booking an appointment tailored to your specific condition.
- To minimise visits, we’ll consolidate tests and appointments where possible.
- If a blood test is required, you will need to collect a form from the surgery and complete the test before your scheduled review.
- As we implement this system, some review intervals may be slightly longer or shorter than 12 months.
- If you do not wish to attend your review, you can decline. However, you will receive one reminder before being removed from that year’s recall list.
Annual reviews are essential for effectively managing your condition, so we strongly encourage you to attend.
